Fibromyalgia

Revised Fibromyalgia Impact Questionnaire

Download the Revised Fibromyalgia Impact Questionnaire (FIQR), a tool you can administer to a patient during an appointment. Review the included instructions on how to administer and score the FIQR so it can be completed in minutes. You can then use the results to help measure the aspects of a patient's health that are believed to be most affected by fibromyalgia.

The scoring for the FIQR is similar to that of the original Fibromyalgia Impact Questionnaire (FIQ). You can use it to compare a patient’s score to his or her previous FIQ results.1

The questionnaire is scored so that an average fibromyalgia patient score is about 50. Severely affected patients usually score 70 or above. The maximum possible score is 100.2
